Registration and Licensing
 

New professionals may want to read up on the instructions and procedures for their Oath Taking as well as how to go about obtaining their Board Certificate and ID .

Registered professionals may opt to Renew their Professional ID or request for Other Registration Services.

 
Initial Registration
  Instructions

Initial registrants should come personally to the PRC Central Office or to the nearest Regional Office to file his/her application for registration and to affix his/her signature in the registry book.

All Registration Certificates shall be claimed by the licensee personally at the Central Office or the Regional Office where the application for the same has been filed. If there is an unavoidable need for a representative, he must present his/her identification paper with picture and a letter of authority from the registrant/licensee.

Requirements

Duly accomplished Oath Form or Panunumpa ng Propesyonal
::
Current Community Tax Certificate (cedula)
::
Two pcs. passport size pictures with name tag (for Oath Form and Certificate of Registration)
::
One pc. 1”x1” picture in plain background with complete name tag (for Registry of Professionals)
::
Two sets of metered documentary stamps (for Oath Form and Certificate of Registration)
::
One (1) short brown envelope with name and profession
 
   
 

Board Certificate & ID

 

Procedures

Log In
Are you a registered MyPRC user, if not, proceed to the MyPRC Registratin section and key in your personal information. Once registered and logged in the PRC system will recognize who you are and automatically fill in the relevant data in your profile to the service form so that you do not have to key in the same information multiple times.

Application
After Logging in, you will be asked how many copies of this certification you want.
::
Confirmation
A confirmation message will be shown, this includes all the order details including fees and next steps. At this point you can go back and change the number of copies or cancel your request.

Payment
Based on the type of payment option chosen, users pay accordingly. Online payment users will get the temporary OR by printing out the payment confirmation page.

Pick Up or Delivery
After the payment is confirmed, the pickup/delivery date will be announced in the transaction section of MyPRC. Pick Up details like ID requirements and the printout of the payment confirmation will be needed
 
   
 

Renewal of Professional ID

 

Manual Procedures

Step 1. Present duly accomplished Application for Professional ID form together with the requirements at:
Unit A – Window 16
Unit B – Window 19
Unit C – Window 30
Step 2. Pay prescribed fees at the Cashier
Unit A – Window 15
Unit B – Window 22
Unit C – Window 26
Step 3. Get your claim slip at the adjacent window.

Step 4. Claim your professional license as scheduled. Please refer to your claim slip for further instructions.


Requirements for Manual Procedure

(a)
Duly accomplished form
(b)
Two (2) pcs. passport size picture: close-up, colored, plain white background, with complete name tag
(c)
Photo/Xerox copy of recent professional ID card
(d)
In case of LOST professional ID card which is still current, the applicant shall submit a notarized affidavit of Loss
(e)
In case of DESTROYED professional ID card, the applicant shall surrender the destroyed card.

Note: Representatives filing the application and claiming the professional ID in behalf of the professional must present Special Power of Attorneyand valid identification of the professional and the representative. PRC-registered professional acting as representatives may transact with PRC upon presentation of Authorization Letter and his/her professional ID.

Step 2. Pay for metered documentatry stamps at the CSC. Please make sure that your photo/xerox copy/ies of documents have metered documentary stamps.

 
Step 3. Pay prescribed fees at the Cashier:
 

Step 4. Present duly accomplished AS together with documents and receive your claim slip at:

 
Step 5. Claim your documents as scheduled. Please refer to your claim slip for further instructions.

3. Replacement Registration Certificate:

Procedures:

Step 1. Accomplish this Action Sheet (AS).

 

Step 2. Pay for metered documentary stamps at the CSC. Please make sure that your AS is stamped PAID.

 
Step 3. Pay prescribed fees at the Cashier:
 

Step 4. Present duly accomplished AS together with the Affidavite of Loss / Damage and receive claim slip at:

 

Step 5. Claim your documents as scheduled. Please refer to your claim slip for further instructions.

4. Verification of license:

If it is a letter requesting verification and does not require a certification, we only reply in the letter form, hence, no fees are charged.
